This form is required to receive funding and/or volunteers for special events and programs scheduled during 2008. If your organization is requesting sponsorship for more than one event, please complete a separate form for each event. If you are seeking merchandise for ditty bags you are required to fill out this form.

 

Please submit completed form no later than 45 days prior to sponsorship
